Alberta Health Services is seeking a skilled and motivated Clinical Pharmacist to join a dynamic, patient-focused team. This role is designed to be performed remotely, with services delivered virtually. Office space may be made available upon request if the successful candidate requires an in-office work environment. You will collaborate with an interdisciplinary team to ensure medications are used safely, effectively, and appropriately while optimizing therapeutic outcomes for patients across Alberta. Working within the EHR-LOCALE study, you will support patients following acute coronary syndrome by leveraging electronic health records to identify individuals with uncontrolled dyslipidemia. In this role, you will connect directly with patients, obtain informed consent, assess current cholesterol management, and implement evidence-based interventions. This includes ordering follow-up lipid testing, prescribing lipid-lowering therapies when appropriate, and completing special authorization or insurance documentation. You will also coordinate follow-ups to ensure continuity of care and improved outcomes. This ambulatory/outpatient-based role involves active collaboration with clinical teams, participation in patient care planning, and ongoing communication with patients and families. The position requires strong critical thinking, excellent communication skills, and proficiency with electronic health systems and drug information resources. You can expect to manage approximately six to ten new patients daily, along with follow-ups as needed. Description:
As a Pharmacist I, you will be responsible for the safe, effective and efficient preparation and distribution of pharmaceutical services. You will manage medication therapy for patients, both initially and on an ongoing basis. In addition, you will provide assessment, intervention and resolution of potential or actual drug-related problems and oversee activities related to compounding and packaging.

Completion of Bachelor of Science in Pharmacy. Current, provisional or transferable registration with the Alberta College of Pharmacy (ACP) as a Pharmacist.
Additional Required Qualifications:Entry-to-practice pharmacy degree (BSc or PharmD) required, along with current registration with the Alberta College of Pharmacy as a Pharmacist, Additional Prescribing Authorization (APA) and current PRACID registration. Demonstrated experience initiating and managing drug therapy, including ordering and interpreting relevant laboratory tests, is essential. Clinical pharmacist experience with Connect Care (ambulatory or inpatient). A minimum of two years of clinical experience managing cardiovascular conditions is required, with a strong focus on acute coronary syndrome and dyslipidemia. Experience optimizing patient access to medications by navigating coverage options and completing special authorization and insurance documentation. Preferred Qualifications:Completion of an accredited hospital residency and/or post-baccalaureate Pharm D program. Experience delivering care virtually, with strong skills in building effective and professional patient relationships through remote platforms. Experience working in an ambulatory clinic or community pharmacy is an asset. At least 1 year of recent experience as a clinical pharmacist in a research study setting within the last 2 years.
