Description:
As a Licensed Practical Nurse (LPN) Personal Support Coordinator, within the Home Care environment, you are part of an interprofessional health care team responsible for the planning, organizing, implementing, monitoring and evaluating of personal support services. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Completion of an accredited Licensed Practical Nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:Minimum 2 years' recent experience as an LPN in Seniors health or long-term care. Previous experience supervising or leading staff. Experience providing direction, teaching, mentoring, or coaching Health Care Aides. Demonstrated experience working within an interdisciplinary team environment.
Preferred Qualifications:Please attach your resume along with all certificates for: Non-Violent Crisis Intervention (NVCI). InterRAI Long Term Care Facilities NZ Enhanced Care Planning training, Supportive Dementia Approaches and Palliative PEARLS.
