Successful completion of an approved Nurse Practitioner program at a minimum of a Master of Nursing level or equivalent. Active or eligible for registration as a Nurse Practitioner with the College and Association of Registered Nurses of Alberta with a practice permit indicating appropriate stream of practice. Experience within Continuing Care streams of Home Care, Designated Supportive Living, and/or Long-term Care Willingness to complete additional education and/or certifications specific to role
Additional Required Qualifications:
Minimum of 5 years of clinical experience as a NP. Minimum 1 year of clinical experience managing palliative care clients (within the last 3 years) in an advance practice role. Minimum 1 year of Seniors Health experience. Experience in at least two (2) care sectors, including Home Care. Advanced clinical & assessment skills. Meditech trained. Competent with MS Office Suite, TEAMs, ZOOM, and tele-health knowledge. Expertise in managing complex behaviors. Proven leadership experience. Access to a vehicle in good repair, and able to travel in inclement weather. Able to carry a 30lb medical bag and/or various supplies.
Preferred Qualifications:
CNA Certification in Palliative Care, or willingness to obtain. Case Management Education. Connect Care Trained. Experience in program planning, education of adults, interdisciplinary practice, and research activities. Excellent interpersonal & communication skills. Ability to work independently & to be flexible to adapt to changing clinical situations. Works collaboratively with all members of the health care team, including patients and their families.
