Nurse Practitioner – Complex Geriatrics

Our Complex Care Support Team is a North Zone team which is currently seeking a Nurse Practitioner (NP) with expertise in geriatrics and dementia to join our team. The Complex Care Support Team is a specialized consultation team which receives referrals for the most complex patients in the North Zone including those with brain injury, dementia and complex medical history. In this role, you'll provide comprehensive care to adult patients across various care settings while closely collaborating with a multidisciplinary team. An understanding of responsive behaviours, medication management of patients with dementia or delirium is an asset within this position. You’ll utilize best practices to optimize patient care and support individuals, families, and primary care providers in the North Zone. As a leader and resource in your field, you’ll mentor fellow team members, lead research initiatives, maintain effective relationships with stakeholders, and engage with clients and families as partners in care. This position involves both face-to-face patient interactions and virtual consultations with clients and the interdisciplinary team. It is also supported by fellow NP’s and Physician specialists. In the future, participation in an on-call rotation for 24/7 virtual NP consultation coverage throughout the North Zone may be required. Description:
As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems. Successful completion of an approved Nurse Practitioner program, with a minimum of a Master's degree. Active registration with the College of Registered Nurses of Alberta (CRNA) on the Nurse Practitioner Registrar, including Provisional Graduate Nurse Practitioner licensure. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:Minimum of 5 years of clinical experience as a Nurse Practitioner. Minimum 1 year of clinical experience managing complex clients with dementia (within the last 3 years). Minimum 1 year of Seniors Health experience. Advanced clinical & assessment skills. Connect Care trained. Competent with MS Office Suite, & TEAMs. Proven leadership experience. Able to work independently in different environments (weather conditions) and must be physically capable of carrying approx. 25-30lbs. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.
Preferred Qualifications:Experience in at least two (2) care sectors, including Seniors Health. CNA Certification in Geriatrics, or willingness to obtain. Case Management Education. Experience in program planning, education of adults, interdisciplinary practice, and research activities. Excellent interpersonal & communication skills. Ability to work independently & to be flexible to adapt to changing clinical situations. Works collaboratively with all members of the health care team.
