Licensed Practical Nurse - Long Term Care Coordinator
Your Opportunity:
Nursing practice takes a Resident-first approach when working with the senior population, focusing on the whole person through advocacy as well as nurturing residents’ identity, integrity, security, growth and joy. Supported by the care manager, the Long Term Care Coordinator will collaborate with Health Care Aides as well as other Licensed Practical Nurses on shift, supporting both as a secondary role to their primary one of administrative tasks. In addition, the Coordinator will also work with the healthcare team consisting of Physicians, Occupational/Physical therapists, Speech Language Pathologists, and other healthcare professionals as needed to promote better outcomes for our residents. The community of High Level, Alberta, located eight hours north of Edmonton and three hours from the border of the Northwest Territories offers a variety of local activities and unique opportunities to experience a part of Alberta and Canada that many do not get to experience. The summertime boasts nearly 24 hours of daylight and the wintertime, while colder than our southern neighbors, offers spectacular Northern Lights and numerous winter activities. As a Long Term Care Coordinator, you will be responsible for leading weekly rounds with healthcare professionals, as well as daily handover with nursing staff. You will also conduct various audits, completing reports, admission and discharge paperwork, in addition to the completion for interRAI (Resident Assessment Instrument), the interdisciplinary instrument that contributes to individualized treatment, targeted nursing care, as well as monitoring the quality of service to continue to improve our residents’ quality of life while in a continuing care home.
Description:
As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team, utilizing nursing processes, critical thinking, problem solving, and decision making skills. You will be guided by your education, experience, and demonstrated skill level, in alignment with applicable legislation, standards of practice, policies, and procedures. You will play a key role in providing safe, high quality, patient and family centred care while reflecting the shared vision and values of the organization.

Completion of an accredited practical nurse education program. Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:Active registration with the college of LPNs and HCAs (CLHA) BLS
Preferred Qualifications:Minimum of two years' experience in a long term care facility
