Licensed Practical Nurse - Intensive Home Care
Your Opportunity:
Reporting to the Area Manager, Red Deer Home Care, you will interact with the team and clients/families to provide quality care to clients in the community. As part of the Intensive Home Care and Community Response team, you will work closely with clients/families and members of the health care team to facilitate early hospital discharge and to prevent hospital admissions when appropriate. The successful candidate will play a key role in timely and appropriate referrals to stakeholders as relevant and/or provide direct care as needed. You may be required to cover other assignments as workload dictates. As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:Excellent communication skills and the ability to work independently in a team environment are needed.
Preferred Qualifications:I.V. certification preferred. Experience in HPT, wound care, palliative care and client teaching is an asset.
