Licensed Practical Nurse - Consult & Treat Team (CAT NURSING)
Your Opportunity:
The Licensed Practical Nurse (LPN), position with the Integrated Home Care (IHC) program provides an exciting opportunity for you to work autonomously to your full scope of practice performing case management functions and providing direct clinical care in a community setting. This integral position supports the IHC program Home Care Consult and Treat Team to provide more consistent coverage of planned and unplanned staff absences to strengthen patient family centered care. In-depth case management, advanced judgment and clinical experience are essential for working in this collaborative multi-disciplinary environment. As a The Licensed Practical Nurse, you will be responsible for coordinating and providing quality care to clients in the community. You will conduct nursing assessments, guide care planning and provide case coordination and direct care in partnership with short term and long-term clients and their families within their own residence or alternative care setting. You will monitor services and ensure they are provided and modified as needed, based on client choice, effectiveness and authorization. It will be crucial to demonstrate proficiency in the following areas: personal and professional accountability, systems thinking, creative thinking, communicating and building relationships, and organizational and time management skills.

Completion of an accredited practical nurse education program. Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:This role requires flexibility, adaptability and rural/remote travelling with your own vehicle. You will be required to operate a laptop computer and mobile phone Manual handling (lifting, lowering, carrying, pushing and pulling) is a regular part of the job. For example: lifting items (approximately 30 pounds) in and out of a vehicle and carrying this weight up and down stairs throughout the day is common within this job. Learning resources and training are provided to safely perform work. Min 2 years experience current LPN experience. Min 3 years experience with Integrated Home Care. Ability to provide clinical service within a continually changing health care environment. Ability to monitor financial accountabilities related to care delivery. Excellent interpersonal and communication skills required. Excellent organizational and time management skills - be able to efficiently organize many details in a time efficient manner. Proven critical thinking and decision making skills. Knowledge of ethical principles & their application in the home environment. Demonstrated ability to operate within an electronic health record. Proven ability to work in a multi-disciplinary team. Good negotiation & conflict resolution skills. Ability to adhere to policies and procedures and legislated requirements regarding confidentiality. Ability to perform all duties in accordance with related legislation.
Preferred Qualifications:Demonstrated assessment skills preferred. Completion of a post-basic Medication Administration Course for LPN’s.
