Located in the beautiful Rocky Mountains with an abundance of outdoor activities at your doorstep, Crowsnest Pass Home Care has an opportunity for a dynamic and enthusiastic individual to join our team. Reporting to the Seniors Health Area Manager, the Licensed Practical Nurse is responsible to provide direct nursing care to clients in various living settings in collaboration with the Community Care Case Manager. As a member of our dynamic team, you will be responsible for understanding and carrying out nursing activities in accordance with the Continuing Care Health Service Standards through promoting the Wellness Independence Model of Care and empowerment of clients.

Description:

As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:

Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

Familiar with guidelines for assignment of health care procedures to Health Care Aides. Comprehensive assessment and nursing interventions for individuals and families. Knowledge of Community resources. Conflict resolution and crisis intervention. Leadership. Time management and organization. Effective oral and written communication. Working independently and as a member of a team. Working in a fast paced; ever changing environment. Working with people in health care environment with professionalism and respect. Negotiating skills with clients, caregivers and families. Family assessment skills. Ability to prioritize and work collaboratively with a multi-disciplinary team. Positive approach and good listening skills in dealing with challenging situations with empathy and sincerity. Valid driver's license and access to a reliable vehicle - requires a car for employment. Able to perform the duties as outlined in the Job Demand Summary which outlines the minimum work level requirements that are required for this position. Copy can be requested.

Preferred Qualifications:

Preference given to candidates with recent Community Care experience (1-3 years). Recent clinical experience functioning at full scope of practice (Physical Assessment, Wound Care/training, Medications, IV therapy, Compression Wrap) preferred. Preference given to candidates with Connect Care experience. Recent clinical experience in psychiatric, geriatric and palliative care nursing preferred. Knowledge of and skills in applying Home Care Nursing Practice Standards.
