If you enjoy a role in providing services to clients in a community-based settings in rural communities, then this position will be of interest to you. The Licensed Practical Nurse (LPN) in Home Care works as a member of an inter-professional health care team utilizing the nursing process to provide direct nursing care to individuals across the life span. The LPN is responsible for her/his nursing practice as defined by the College of Licensed Practical Nurses of Alberta (CLPNA). In collaboration with the Registered Nurse Case Home Care Manager, the LPN is responsible to assess, plan, implement and evaluate the needs of Home Care clients and their caregivers/families. Based on the Continuing Care Case Management Competency Framework you demonstrate ability in the areas of personal and professional accountability, systems thinking, facilitating creative thinking, communicating and building relationships. Using a person-centered approach you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care to individuals across the life span. In collaboration with the client/family you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization. There is also an expectation to be a mobile worker, including caring for clients in their homes or in the community. Please note this position is approximately 64 km SW outside of Calgary City Limits.

You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:

Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

Completion of a post-basic Medication Administration Course for LPN’s. You will be responsible to carry and move equipment, use and operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling upwards to 50 Ibs.) as a regular part of the job. Learning resources and training are provided to safely perform work. Preferred Qualifications:

Minimum of two years current nursing experience. Recent work experience in a community-based setting. Home Care experience preferred.
