Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. Supportive Living (SL) is a Program that provides an integrated system of health and personal support services to clients based on assessed need. SL has an opening for a Case Manager Educator (CME) within the Education and Practice Development portfolio. The CME facilitates, coordinates, and supports the implementation of projects and initiatives within the program, which is influenced by orientation, professional development, continuing education, changes in policy, procedure, protocols, and technology. The CME acts as a role model and resource to staff for professional development, clinical competency, and case management skills. This role focuses on providing comprehensive resources to SL healthcare professionals, ensuring a holistic approach to client care. Responsibilities include developing and delivering educational materials that emphasize the wellbeing of clients. This position works with the interdisciplinary team and consults with managers to determine learning needs and operational requirements. Supports new staff and encourages best practice by group and one on one coaching and mentorship. Mentoring entails review of another staff members work and providing appropriate feedback and support to address any areas for improvement. Attends site meetings and case conferences with staff. Chairs meetings as required. Influences and supports a culture of professional growth. Assists with the assessment of education needs of staff and the identification of appropriate learning resources.

Description:

As a Health Educator, you will promote existing courses, as well as develop, coordinate and deliver educational activities to support staff learning based on a patient-centered care model. You will collaborate with Health Educators from all the Zones and relevant initiatives.

Required Qualifications:

Active or eligible for registration and practice permit with the applicable college.

Additional Required Qualifications:

Two years recent experience in the community sector (within the last 4 years). Two years’ experience working as a Case Manager in community. Strong computer skills (Microsoft Office) are necessary. Experience with development, implementation, facilitation and evaluation of education materials and programming is required. Proven exemplary communication (both written and oral), interpersonal and organization skills are essential. Demonstrated leadership and teamwork abilities, along with the ability to work effectively in a self-directed environment are vital. Travel among sites is required, thus must have a reliable vehicle. At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands-on assessment. You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet. The successful candidate will work in a congregate setting, with multi-building setup. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry medical bags, which weigh approximately 25-30 lbs.

Preferred Qualifications:

Current working knowledge of Connect Care and inter-RAI, Meditech an asset. Recent (within the last 2 years) Supportive Living experience. Community educator experience is preferred, clinical educator experience an asset. Post graduate studies or certification in adult education, gerontology, chronic disease management and/or palliative care would be an asset.
