Active or eligible for registration with applicable professional association or college.
Additional Required Qualifications:Current registration as a Registered Nurse (RN) in good standing, with a minimum of two (2) years of clinical experience, preferably within Home Care, Continuing Care, Transition Services, or Home and Community Care settings. Demonstrated knowledge of home care operations, continuing care systems, and community-based resources. Proven excellence in both written and verbal communication, with the ability to engage effectively with clients, families, and interdisciplinary team members. Highly self-motivated, adaptable, and flexible, with the ability to thrive in a fast paced, dynamic environment. Strong team player with the ability to collaborate, problem-solve, and contribute positively to team culture. Demonstrated ability to navigate and work efficiently across multiple computer programs while conducting client assessments by phone. Strong working knowledge of Connect Care and Microsoft Office applications. InterRAI competency and completion of AHS Case Management training (or willingness to obtain).
