Join an amazing team of professionals that are responsible for the day-to-day operations and activities related to client's access to Continuing Care Homes in the Central Zone. The Continuing Care Home Coordinator uses a collaborative approach to review all applications for Continuing Care Homes, as well as manage the Central Zone wait list according to guidelines and policies in place. The coordinator will also act as a resource for facilities and community health professionals, revise priorities on a continuous basis and coordinate the placement of individuals in Continuing Care Homes in the Central Zone.
Description:
As a Continuing Care Counsellor (CCC), you will be working closely with interdisciplinary professionals with a variety of backgrounds where you will provide a wide variety of coordination activities to support patients and their families to access Continuing Care living options. Using a collaborative approach, you will review applications, manage continuing care waitlists and coordinate placement processes. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures, while reflecting the shared vision and values of AHS.

Active or eligible for registration with applicable professional association or college.
Additional Required Qualifications:Minimum 2 years of clinical experience working as an RN/RPN preferably in Home and Community Care or a Continuing Care Home environment. Knowledge of home care, continuing care, and community resources. Demonstrated excellence in written and verbal communication. The successful candidate must have the ability to work in a fast-paced environment, be highly self-motivated, adaptable, flexible and a team player. Have the ability to work in multiple computer programs while completing phone assessments and strong working knowledge of Connect Care, and Microsoft office products.
Preferred Qualifications:Experience with Inter-Rai Contact Assessments and case management training is preferred.
