ERHL is an Indigenous-focused primary health care clinic that offers wraparound healthcare to First Nations, Inuit, and Métis individuals and families. The goal of ERHL is to provide access to care that is safe and respectful of cultural beliefs. ERHL is made up of an interdisciplinary team providing medical support from physicians, nurses, and specialists. Connection to traditional FNMI healing and cultural services is supported by Elders/Knowledge Keepers. Outreach staff help clients with social work and nursing support for crisis and resource navigation, case management, and advocacy. The vision of ERHL is to have healthy Indigenous communities that are connected and empowered to access healthcare. The Community Liaison Worker (CLW) will be working within ERHL, and also provides outreach to clients in community; provides support, resources, referrals, and advocacy to individuals and families, to assist in overcoming any access barriers; helps build relationships with Indigenous communities and organizations within Calgary and area; supports complex clients in navigating the health system, and promote the health and wellbeing of clients experiencing social, financial and/or health challenges; requires flexibility and adaptability within a complex and rapidly changing environment that may involve working with some clients in crisis who may be dysregulated and/or emotionally distressed; provides consultation and assistance to the health care team regarding appropriate accommodations of Indigenous health practices and beliefs.

Description:

As a Community Liaison Worker (CLW), you will provide services to clients seeking Mental Health assistance. You will provide one-to-one support to a diverse client population in a variety of Mental Health settings according to the client's individual service plan, and/or treatment plan in accordance with the client-centered approach. You will participate in assessments to determine access barriers and the support needs for clients accessing Mental health services. You will focus on assistance with collaboration and service coordination with additional support/resource modeling in a variety of settings. A working knowledge of mental health issues is required.

Required Qualifications:

Completion of bachelor's degree in a health related field. Active or eligible for registration with applicable professional association or college. Minimum two years' experience working within Mental Health. Current Basic Cardiac Life Support - Health Care Provider (BCLS - HCP). Possession of a valid driver's licence and access to a motor vehicle in good repair for work-related travel.

Additional Required Qualifications:

Please note, due to system limitations, this role differs from the Position Summary. Minimum 3 years experience working with Indigenous populations. Experience in advocating for clients by coordinating the efforts of all internal and external parties to meet client and family psychosocial and instrumental needs such as housing, food, transportation, income support and financial management. Knowledge of resources specific to Indigenous populations is required. Minimum 3 yrs recent community or primary care experience. Experience working with complex high-risk clients, addictions, & mental health with potential exposure to verbal aggression. Understanding & training with Anti-oppressive practice, Stages of Change, & Trauma Informed Care. Demonstrated attention to detail & accuracy. Strong communication skills with ability to relay patient information to other healthcare providers in a timely, concise, accurate manner in order to facilitate treatment decisions. Ability to balance numerous demands & conflicting priorities. Established relationships with Indigenous serving agencies, Nations, & supports in Calgary & surrounding area. Experience with an EMR system (Connect Care) and demonstrated aptitude with computers and technology. Extensive knowledge of the benefits and financial supports available to Indigenous clients is necessary. Experience supporting clients with AISH & other financial support applications. Experience with the Child & Family Services system.

Preferred Qualifications:

Working knowledge of Indigenous culture, history, and social determinants of health with a strong working knowledge of client experienced barriers to health access. Comfortably in addressing diverse social determinants of health, substance misuse, intergenerational trauma, adverse childhood experiences and systemic factors that perpetuate poverty, homelessness and toxic stress. Strong understanding of harm reduction, anti-oppressive and trauma informed practice, and the effects of colonization.
