Community Health Representative - Healthier Together
Your Opportunity:
This position will support planning and implementation of the Healthier Together Initiative. Healthier Together is an Alberta-based population health promotion system with communities, workplaces, school and health services. It is a scalable way of working that uses a practical 5-step approach to bring people together, understand the local context, prioritize and plan, implement evidence informed strategies, evaluate and scale and spread successful initiatives across the province. The Community Health Representative - Low German will collaborate with diverse stakeholders, including AHS staff, community partners and the Low German community.
Description:
As a Community Health Representative, you will work in partnership with community health staff in the provision of screening programs, health education and communicable disease control, and enhance the community capacity through strong community partnerships.

Completion of Grade 12 or equivalent.
Additional Required Qualifications:Demonstrated ability to communicate fluently in the Low German language (Plautdietsch). Knowledge of and sensitivity to Low German Mennonite culture. Demonstrated experience working and collaborating with community groups and agencies. Knowledge of health care systems and health promotion/healthy living strategies and processes. Proven organizational and time management skills. Familiarity with health and social services and resources in Southern Alberta. Strong interpersonal and communication skills to engage effectively with key populations and community partners. Proficiency in public speaking, health education and presenting information clearly. Demonstrated ability to work independently and as part of a multidisciplinary team. Proficiency in the use of Microsoft programs (Word, Outlook, Excel, PowerPoint, Teams, OneDrive).
Preferred Qualifications:Proficiency in the use of Connect Care.
