Care Manager - Complex Care Support Team

As the Care Manager, Complex Care Support Team, you will lead a North Zone interdisciplinary healthcare team that provides expert consultation service for clients in the Continuing Care Stream (or referred for service in the Continuing Care Stream) where behavioral needs cannot be met with a usual Continuing Care level of service. The team provide consultation services in locations such as: acute care, continuing care, and home care settings to provide specialized assessment and intervention on a consultative basis. The team works collaboratively with health care professionals, consultants, primary care physicians, patients, and families along the continuum of care to ensure the provision of quality care services that follow evidence-based best practices. This position is based out of Hinton, Alberta, but consideration will be given to other locations within North Zone. The position is largely a virtual role, but at times will require so in zone travel.

Description:

The Care Manager is a key leadership role within NZ Seniors Health, reporting to the Manager, Specialty Teams, NZ. The position provides direction and leadership aligned with the ALA business plan, performance measures, and team goals. Responsibilities include fostering strong stakeholder relationships, supporting team development and change management, providing education on policies, processes, and services, and representing the North Zone Complex Care Support Team on committees and working groups. The Care Manager reviews complex cases, provides guidance and recommendations, attends conferences as needed, and facilitates weekly team meetings to address concerns, complex cases, and vacancy management. The role promotes collaborative, patient-first, client-centered care while supporting the team in managing complex client issues and achieving positive outcomes. The position evaluates performance measures, identifies opportunities for improvement, and responds to urgent requests related to team statistics, information, policies, and processes. As an advanced Continuing Care subject matter expert, the Care Manager supports clinical operations, program development, and complex problem-solving through specialized consultation services. The role ensures coordination and engagement across multiple care streams, including Addictions and Mental Health, Public Health, Acute Care, Clinical Quality Improvement, EMS, Allied Health, and Social Work. Required expertise includes continuing care and seniors’ health programs, gerontological nursing, palliative care, psychogeriatrics, case management, clinical informatics, and complex behavioural management.
