Your Opportunity:

Come join our amazing Calgary Zone Rural South Team as a Home Care Manager for Okotoks and Diamond Valley and surrounding area. The Home Care Manager will be responsible for providing leadership to an interdisciplinary team of healthcare professionals and Health Care Aides dedicated to providing leading care to Albertans. Your leadership, direction and organization of this department will be on site to support of the vision, mission, and business plan of AHS. You will be making key decisions and establishing and contributing to the achievements and goals of the Seniors Health portfolio. As the Home Care Manager, you will be responsible for developing and maintaining strong working relationships with key stakeholders, facilitating change management, promoting team development, teamwork and coordinating staffing and clinical work as identified by the Area Manger.

Description:

Under the direction of the Rural South Area Manager, the Care Manager provides clinical and operational leadership to the Rural South Integrated Home Care Team, specifically supporting the sites of Okotoks and Diamond Valley. This role supports an integrated interdisciplinary team of mobile professionals and Health Care Aids in the community. There is a strong focus on client centered care, the development and monitoring of staff and material resources. Supporting staff is fundamental to this position. Strong communication skills facilitate the internal function of this role and promote as well as maintain external partnerships essential in the delivery of care. The Care Manager acts as a change agent facilitating the evolving nature of the Integrated Home Care Program. Identifies staffing needs required to effectively and efficiently operate. Participates in the entire recruitment process. Encourages and supports staff to identify learning needs; promotes continued learning opportunities for professional growth. Monitors staff performance, provides immediate, constructive feedback to facilitate ongoing quality performance. Completes staff performance reviews. Fosters internal/external information exchange related to areas of responsibility with various stakeholders. Facilitates/leads regular team meetings providing staff with current guidelines and process information impacting clinical/case management practice. Fosters client centered care, recognizing unique client situations and complexity of care delivery in the community. Promotes a safe and respectful workplace.

Required Qualifications:

Bachelor's degree in field of clinical practice. Current registration and in good standing with registration college body.

Leadership skills, including demonstrated ability and comfort with decision making responsibilities, coaching, and teaching, and the ability to inspire and build confidence in others. Demonstrated ability to manage change. Ability to manage human, financial and physical resources within an operating environment. Proven ability to foster partnerships and to achieve organizational goals within an organization and ideally managing within a unionized sector. Strong track record in establishing and maintaining effective working relationships with internal and external stakeholders. Effective leadership abilities combined with a strong commitment to operating within a team environment and the ability to motivate and inspire other to achieve common goals. Excellent analytical and decision-making skills. Ability to foster a climate of cooperation amongst, and build solid relationships with public agencies, government, committees, and other partners.

Preferred Qualifications:

Minimum of two years of management experience within a unionized environment. Home Care experience an asset.
