Completion of Grade 12 or equivalent.
Additional Required Qualifications:Grade 12 graduation supplemented by commercial/business courses. Minimum of 2 years related experience in a current/past experience in Access/Transition Services. Medical terminology required. Must have excellent interpersonal, communication and organizational skills in order to work with a multidisciplinary team. Must have outstanding prioritization skills to analyze and respond to client/ community needs accordingly, and cope with frequent interruptions. Ability to multi-task. Excellent knowledge of the English language, both written and oral. Accurate and proficient keyboarding skills of 40-50 words per minute required.
Preferred Qualifications:Demonstrates conflict resolution skills, and ability to work collaboratively within the Access and community programs. Demonstrates proficiency with Community Care Information Systems. Required to perform up to 6 hours of keyboarding and written documentation per shift while navigating a variety of software programs (Pathways, PARIS, Netcare, PD, HomeFirst). Past experience working with clientele/customer service.
