Completion of a bachelor's degree in a health related field. Active or eligible for registration and practice permit with the applicable college.
Additional Required Qualifications:Five (5) years relevant healthcare experience with one (1) year Seniors Health experience. Home Care, Seniors Health, dementia care, discharge planning, and experience with placement or working in DLO environment. Please note, this facility provides Long Term Care (LTC) or Designated Supportive Living (DSL) services and by Order of the Chief Medical Officer of Health, many AHS employees are restricted to work only at a single LTC/DSL site for the remainder of the pandemic. Speak to the hiring manager who will confirm if, as the successful candidate in this position, you will be restricted to work at only one LTC/DLS site in Alberta.
Preferred Qualifications:Knowledge of computer applications (Netcare, Meditech, MS Office); demonstrated competency with RAIHC; AHS McMaster Case Management; Supportive Pathways Training and knowledge of cognitive status and functioning; conflict resolution and negotiation skills; experience with clinical assessment; knowledge of applicable legislation, programs and community resources; and demonstrated ability to navigate the healthcare system and advocate for clients/families.
